Please understand that payment of your bill is considered part of your treatment. I do not participate with any insurance programs, including
private insurance, medicaid or medicare. As an out of network provider, I will provide insurance print outs and clinical case reveiws for direct reinbursement to you.

Full payment is due at the time of service.

A 24 hour notice must be given for a cancellation. Failure to provide 24 hour notice means full payment of fee is required.
